M3P doesn't change what you owe — it's cash-flow smoothing, not a discount. You still pay $1,586.25 total either way; M3P just spreads it evenly across the remaining months instead of billing it all at the pharmacy when you fill a prescription. This is a simplified even- spread projection — your real monthly bill can vary if your drug costs land unevenly across the year.
Premiums never count toward the $2,100.00 cap — only your deductible, copays, and coinsurance for covered drugs do.

How the 2026 Part D benefit actually works, phase by phase.
Medicare Part D used to have no ceiling on what beneficiaries could spend out of pocket for covered drugs in a year — for people on expensive specialty medications, costs could run into the tens of thousands of dollars annually. The 2022 Inflation Reduction Act rebuilt the benefit from the ground up, phasing in the first-ever hard annual cap: $2,000 in 2025, and $2,100 for 2026.
The three phases
Every Part D enrollee moves through the same structure each year, though not every plan structures the first phase identically:
- Deductible: up to $615 in 2026, paid in full before coinsurance starts. Not universal — some plans charge less or waive it for some or all tiers.
- Initial coverage: 25% coinsurance on covered drug costs, continuing until your total out-of-pocket spending (deductible plus coinsurance) reaches $2,100.
- Catastrophic coverage: once you hit $2,100, you pay $0 in cost-sharing for covered drugs for the rest of the calendar year.
One detail that trips people up: the $2,100 threshold is measured in what YOU pay — deductible, copays, and coinsurance — never your monthly premium. Two people with identical drug regimens but very different premiums reach catastrophic coverage at exactly the same point.
The Medicare Prescription Payment Plan (M3P)
Also created by the 2022 IRA, M3P is an opt-in program that lets you pay $0 at the pharmacy and get billed monthly instead, spreading your remaining out-of-pocket costs evenly across the months left in the year. The single most important thing to understand about it: M3P is not a discount. It doesn't reduce your total drug spending by a single dollar — it only changes when you pay it. If your annual out-of-pocket total is $2,100 and you enroll in January, your flat monthly bill is $2,100 ÷ 12 = $175.00; enroll later in the year and that same total gets divided across fewer remaining months, so each payment is larger. It's most useful for people who expect large drug costs early in the year and would rather not pay a big lump sum upfront.
This calculator models M3P as an even, idealized spread of your projected annual total across the months remaining from your enrollment month — the same simplification implied by CMS's own worked examples when projecting a flat monthly estimate. Your actual monthly bill can differ if your real drug costs land unevenly across the year, since CMS's real formula responds to your month-by-month spending, not just an annual estimate.
